CLIENT INFORMATION SHEET AND CONSENT FORM The following information pertains to EMDR service provided by AntonyJohn’s Consulting. APPOINTMENTS Appointments are usually 80-90 minutes. The number of appointments required will vary according to the severity and complexity of trauma, 12 sessions minimum. This will be discussed with you at the end of the first session and reviewed accordingly. FEES The Australian Psychology Society (APS) recommended rate is $300 per hour. This practice charges $200 per EMDR session online, $250 in person, and the rebate is $98.95. Fees are reviewed annually. Payment is made via direct bank transfer or credit card. PLEASE NOTE: Rebates are only available for individuals a GP has referred under a Mental Health Care Plan. You can pay the gap or pay the full amount and process the rebate yourself. CANCELLATION POLICY At least a 24-hour notice is required to cancel an appointment, or a fee equivalent to the cost of the session applies. If a late cancellation, non-attendance or session fee is not paid, future appointments will be suspended until payment is received. TELEHEALTH APPOINTMENT The following is the procedure when you cannot attend the appointment in person. The consultation will be conducted via telehealth (video or telephone). Your psychologist will send you a link for your appointment either the morning of your appointment or the night before your appointment. No information about the sessions will be released without your permission. There are a few exceptions to this: ● If there is a concern that you or someone else is at imminent risk of harm; ● If you have given written consent to share information with someone else, and if ● The court issues a subpoena requesting your case files. Written permission will be sought if you want information to be provided to another health professional or a third party.
